RN Case Manager - Weekend days

The Case Manager Registered Nurse (RN) is responsible for offering integrated and extensive care to patients. With moderate to general guidance, they assess patient needs, engage in care coordination, carry out research, harmonize information, and evaluate care arrangements. Essential Job Function

• Reviews patient instances to understand their necessities and design custom care blueprints.
• Processes patient care documentation meticulously to meet regulatory standards and ensure precision.
• Engaging in ongoing research efforts to remain current with evidence-based practices and healthcare recommendations applicable to the designated patient group enhances the quality of care plans and interventions.
• Assesses and appraises the success of healthcare plans, supervising patient results and modifying as required to maximize patient welfare and attain desired targets.
• Executes any other responsibilities given.

Education

• Required: Associate's Degree Preferred: Bachelor's Degree

Experience

• Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations

Licensure/Certification/Listing

• Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
