As a Registered Nurse in Home Health, you will play a pivotal role in delivering and directing nursing care to patients in the comfort of their homes, following physician guidelines and adhering to all applicable laws, regulations, and agency policies. You will coordinate comprehensive care plans alongside fellow healthcare professionals to ensure the highest quality of patient care.

Key Responsibilities:

• Collaborate with physicians and the interdisciplinary team to develop, implement, and update individualized patient care plans while completing comprehensive nursing assessments.
• Provide high-quality clinical care, supervise home health aides and LPN/LVNs, and ensure compliance with federal and state regulations during required supervisory visits.
• Maintain timely and accurate documentation of all patient care in accordance with agency policy and communicate efficiently with patients, families, and healthcare staff.
• Assist in orienting and training new staff; engage in performance improvement, patient safety initiatives, and reduce avoidable hospitalizations.
• Exemplify professionalism through punctuality and a positive attitude, while embracing ongoing learning and agency standards, including participation in on-call and weekend rotations.
• Deliver care without bias, welcoming clients from all backgrounds regardless of their age, race, creed, color, sex, disability, or financial status.

Required Qualifications:

• Current and unrestricted RN licensure in your state of practice.
• Valid Driver's License, vehicle insurance, and access to reliable transportation.

Preferred Qualifications:

• At least 1 year of clinical experience.
• CPR Certification or the ability to obtain it within 90 days of hiring.

Compensation is commensurate with factors including local labor markets, experience, and education. A comprehensive benefits package is offered, including an equity stock purchase plan and 401k contributions (subject to eligibility). Salary for this position ranges from $82,016 to $123,024 annually based on full-time employment.
